Job summary We are looking for individual who would like to be part of a dynamic and forward-thinking team. An exciting opportunity has arisen within Romsey Community Care Team to appoint an enthusiastic RNA (Band 4) or an experienced Health care support worker that is working towards this qualification. The role will include visiting patients at home to provide nursing care under the direction of registered nursing colleagues. The focus of the role is to support admission avoidance, to promote independence and to assist in the effective management of patients with long term conditions. The post would suit someone interested in integrated working and wanting to achieve the best for their patients. They will work within a multi -professional team to provide individualised patient care which is made up of Nursing and therapy staff, Frailty Practitioners, social workers, and GP's. We are looking for a motivated individual, who is dynamic and keen to work as part of a team. The successful candidate will need to have good communication skills and the ability to work well as a team member. If currently working towards the RNA qualification evidence must be provided at the interview of this. If successful, the pay will be awarded at the lowest point of the band 4 pay scale. The next point of the pay scale will be granted in the timeframe stated in the NHS scale if obtaining the qualification has been achieved, otherwise the next point will be awarded on achievement. Main duties of the job To contribute to the provision and monitoring of high quality individualised nursing care in accordance with agreed policies and procedures Work independently under the leadership of the Registered Nurse to, plan, organise and deliver clinical care in line with service and patient needs. Nursing Associates are equipped with the appropriate level of knowledge, understanding, skills, attitudes and behaviours and will work within their scope of practice in accordance with the Nursing & Midwifery Council (NMC) code of conduct and standards of proficiency for Nursing Associates.
