An opportunity for 2 specialist Physiotherapists to join our Stroke & Neuro teams on 12-month Fixed Term Contracts
We have 5 rotations in Acute/Rehab & Community settings, based at St Peter’s Hospital, Woking and Walton Community Hospitals, and in patients’ own homes:
1. Hyper acute and Acute Stroke Unit
2. Stroke Rehab
3. Neuro-rehab - B radley Unit: regional unit for patients with conditions such as TBI, MS, SCI and GBS
4. Community Neuro-rehab - with Central Surrey Health (CSH), providing treatment and supporting long-term management

You'll need senior experience in stroke or neurology and preferably have worked in community settings; also a broad grounding in a variety of fields including acute hospital experience and different specialties, e.g. older people, orthopaedics, medicine.

Main duties of the job
1. Undertake advanced assessments, formulate clinical diagnoses, problem lists and patient led goals, to provide appropriate treatment for patients with neurological conditions which may be acute or sub-acute, complex and / or chronic presentations
2. Work as an autonomous practitioner to undertake a significant specialist clinical caseload, working without direct supervision
3. Contribute to board rounds or problem patient sessions to ensure that patients get the right treatment, in the right place at the right time
4. Work as part of a multi-disciplinary team, putting patient care and patient experience first

7. Facilitate onward planning and safe discharge from hospital

Detailed job description and main responsibilities
1. Undertake specialist patient assessments, including those with diverse or complex presentations/multipathologies and assess patients jointly with other members of the multidisciplinary team
2. Use sound investigative, analytical and clinical reasoning skills, manual assessment techniques and diagnostic procedures to assess and develop treatment programmes
3. Devise individualised physiotherapy management and / or treatment plans, using a range of treatment/ management options, to include patient education and exercise and use of groupwork/ classes in other settings, also including video and telephone consultation
4. Formulate accurate prognoses and recommend best course of intervention, developing comprehensive, timely discharge plans, liaising with community rehabilitation teams and intermediate care teams
5. Evaluate patient progress, reassess and alter treatment programmes if required
6. Evaluate the outcomes of your interventions
7. Demonstrate the physical ability to carry out physiotherapy assessments and interventions, including manual physiotherapy techniques and therapeutic handling. This post includes frequent light to moderate physical effort
8. Keep up to date with developments, analysing current research and discussing and implementing changes to your clinical practice

17. Work as part of the physiotherapy team and wider MDT to provide an efficient and effective service
18. Deputise for the Team Leader when required
19. Effectively plan the daily management of the patient caseload by prioritising clinical work, responding to urgent referrals and occasionally unpredictable work patterns
20. Prioritise clinical work and managerial work, and balance other patient-related and professional activities as required
21. Communicate effectively with all disciplines involved in patient care, both within and outside the organisation, to ensure effective multidisciplinary working
22. Communicate complex and sensitive information to patients, carers and other staff including imparting unwelcome news relating to their rehabilitation potential and gaining cooperation for treatment and valid consent
23. Use influencing and motivational skills to encourage patients to manage their own conditions
24. Communicate any changes in care plan or treatment approach to other physiotherapists as necessary
25. Ensure timely and effective communication with the Clinical/Therapy Leads on all professional matters
26. Maintain accurate records in line with the Chartered Society of Physiotherapy standards as well as Trust and service standards/policies. Ensure that records of directly managed staff also comply with these standards
27. Attend relevant board rounds, meetings and case conferences to ensure appropriate management and effective and timely discharge of patients
28. Attend staff meetings and actively participate ensuring understanding of information
29. Assess patients’ understanding of treatment proposals, and be able to work within a legal framework with patients who lack capacity to consent to treatment
30. Use a range of verbal and non-verbal communication tools to communicate effectively with patients to progress rehabilitation and treatment programmes. This may include patients with comprehension or communication difficulties, e.g. patients who may be deaf, blind, or be unable to accept diagnoses or presenting condition, or who have cognitive and behavioural problems, and those for whom English is not their first language
