We are looking for an enthusiastic Band 5 Community Staff Nurse to work with the OOH District Nursing Service 3 evenings per week, from 17:00 to 23:00.
You will join a team of nurses delivering high-quality care to patients who are housebound, caring for those with long-term conditions and complex health needs, while developing your skills and knowledge within an integrated healthcare structure.
You should be able to demonstrate your ability to make clinical decisions to provide evidence-based practice and care planning in response to clinical assessments.
Main Duties of the Job
1. Perform holistic assessments of patient nursing needs.
2. Plan, assess, implement, and evaluate care delivery according to changing health needs, utilizing prescribing skills.
3. Ensure practice is evidence-based through undertaking audits and critically appraising literature.
4. Support the District Nursing team by undertaking responsibility for a delegated caseload.
5. Liaise with the District Nurse team leader to ensure information sharing related to patient care and continuity of care.
6. Mentor, teach, and assess junior staff.
7. Liaise with all healthcare professionals and other agencies for effective service delivery.
8. Involve patients and carers in the planning and delivery of care.
9. Maintain accurate records in accordance with NMC Standards.
10. Participate in health needs assessments of the practice population.
11. Work independently and solve problems as they arise.
12. Work on a rota basis to facilitate weekend working across the district nursing teams.
