You will be expected to make autonomous professional decisions, applying your knowledge and skills in physical examination, applied pharmacology, and evidence-based prescribing. Key Responsibilities: First Point of Contact:Assess and manage a broad range of patient conditions across all ages, using triage models, including online triage, face-to-face consultations, and home visits. Direct Patient Access:Provide direct access for patients, planning investigations, treating, and referring as appropriate for undifferentiated and undiagnosed health issues. Collaboration with the Digital Team:Work with the PCNs digital team to evaluate and implement various IT and digital solutions to enhance patient care. Professional Autonomy:Make clinically autonomous decisions, including physical assessments, prescribing, and managing patient care plans based on evidence-based guidelines. Achieving IIF Targets:Contribute to achieving IIF (Improvement and Impact Fund) targets as part of the Network Contract DES, focusing on quality care and patient outcomes. Patient Record Management:Maintain accurate and up-to-date patient records, storing details of referrals and care pathways. Training and Development:Assist with the delivery of training to colleagues within and outside the profession, in consultation with GPs, the nursing team, and other primary healthcare professionals. Protocol Development:Regularly review and update PCN protocols and guidelines to ensure best practice. Collaboration:Develop and maintain effective working relationships with key stakeholders, including GPs, consultants, district nurses, social services, Trusts, and the voluntary sector.
