This role replaces our current Clinical Pharmacist who is retiring. Main duties of the job To be responsible for medicines management within the practice and manage the team responsible for delivery. To be part of the practice management team. To be responsible for medicines safety within the practice. To advise on appropriate prescribing in general practice and provide education on appropriate new medication and guidelines. To lead on delivering the medicines element of the Local Incentive Scheme. This will include delivering Structured Medication Reviews for older patients with polypharmacy. Assist with QOF where appropriate. To provide face to face and telephone appointments for medication review. To provide face to face and telephone appointments for chronic disease management as appropriate. To take the lead for the practice in one or more clinical areas. Liaise with local Community Pharmacies to encourage collaborative working. Implementation of Local/National Guidelines and Formulary Recommendations As appropriate Cost Effectiveness Support the delivery of QOF and other incentive schemes. Care Quality Commission Work with the general practice team to ensure the practice is compliant with CQC standards.
