The successful candidate will be based within the Integrated Discharge Hub - Hospital Social Work Team delivering a service to patients in St John's Hospital to help facilitate timely discharge from hospital and to minimise discharge delays.
The Integrated Discharge hub is an inter-agency multi-disciplinary team made up of hospital social workers, community care officers, reablement staff, discharge facilitators, AHP staff as well as other representatives from different services.
This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First.
The worker must have the ability to work as part of a team in a pro-active manner identifying opportunities for Home First whilst ensuring the patient has the appropriate level of support to discharge from hospital.
Tasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals and agencies as necessary to meet need. The worker will be required to have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments.
The worker will be required to undertake essential training including in adult support and protection training.
The worker must have the ability to travel throughout West Lothian.
Before confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland.
The skills and knowledge candidate must have:
1. SVQ 3 or HNC in Social Care or equivalent or relevant experience in a social or health care setting is essential.
2. It is desirable for the candidate to have relevant assessment and care management experience in a social work practice team.
This position requires registration with the SSSC. The successful candidate will have six months from the date of commencement of this post to become registered with the SSSC.
SSSC stipulate you must submit your application within three months of your start date.
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
