Main duties of the job
In this post, the post holder will act as a clinical expert and be instrumental for ensuring that an integrated falls strategy is continued at Medway NHS Foundation Trust which will meet National and Local drivers in reducing falls and falls with harm within the hospital and local community.

Job responsibilities
1. Lead the Falls Service across specified areas of responsibility to ensure a unified approach to care, consistent standards of care and patient access.
2. To lead on the development of appropriate policies and standards relating to services auditing results and responses.
3. To review local services for falls and fractures to identify gaps and to establish a baseline for service delivery, including the identification of multi-agency resources currently supporting Falls and fractures.
4. Influence the care provided by other Health and Social care professionals through provision of guidance and involvement in formal teaching.
5. Demonstrates a highly developed specialist knowledge within own discipline underpinned by current evidence-based practice acting as a reference for colleagues and other agencies.
6. To actively engage with clinicians and all key stakeholders and to foster good working relationships to ensure effective partnership approach to achieving multi agency outcomes.
7. To coordinate and manage the ambulatory falls care pathway, triaging patients onto an appropriate clinic or activity programme.
8. The post holder is responsible for supporting the senior sister charge nurse and multi-professional teams in the day to day delivery of care, ensuring optimal high quality fall prevention and falls management is delivered and continuously monitored.
Person Specification
Qualifications
* BSc / Degree or equivalent qualified Registered Nurse
* Masters level of qualification or professional knowledge will be supplemented by additional training to Masters level equivalent
* Evidence of ongoing continuing professional development in relevant area of speciality supplemented by specialist clinical, managerial training and CPD.
* Qualified Instructor for Chair based Exercise or understanding of principles.
Knowledge
* Extensive Knowledge of issues, guidelines, policy and best practice in relations to falls prevention and management.
* Understanding of the role of other NHS bodies as well as partner agencies statutory, independent and voluntary sectors.
* Up to date knowledge on current falls/fracture prevention research.
* Understanding the management of osteoporosis and bone health.
Experience
* Considerable experience of working with this client group.
* Autonomous practice working.
* Teaching people (patients and professionals). Effective Leadership skills.
* Ability to work within multi-professional team.
* Delivering training to others. Managing supervising others.
Skills
* Excellent written and oral communication skills.
* Ability to communicate with the general public.
* Ability to project a professional manner and approach in clinical areas.
* Ability to present strategic plans to board members when invited to Patient Safety Committee.
* Ability to formulate reports on the numbers and factors surrounding falls, on a monthly basis.
