An exciting new opportunity for an experienced Occupational Therapist has arisen to work as part of a therapy team at Carlton Court intermediate inpatient care unit based in Lowestoft.

The post holder will be expected to provide specialist clinical skills to manage a caseload of patients using evidence-based practice in order to achieve their goals.
This post is 22.5 hours per week.
Main duties of the job
The post holder will provide a high standard of care as part of a multi-disciplinary team to patients within the intermediate care service and patients' own homes. The post holder will be working as an autonomous practitioner applying a high level of clinical reasoning to ensure that individualised care is delivered safely.
The post holder will be responsible for delivering an Intermediate Care service for patients with highly complex needs within a ward-based setting; however, there may be a requirement to work across a wider community setting. The emphasis of the service will be reablement of patients in order to promote:
1. Faster recovery from illness.
2. Prevention of unnecessary acute hospital admissions.
3. Prevention of premature admission to long-term residential care.
4. Support for a timely discharge from hospital.
5. Maximize independent living.
Develop and maintain clinical skills in line with the role's job description and specific professional registration.
Provide individualised care plans for a complex patient caseload and ensure that any delegated workload is completed by staff with the appropriate competencies.
Job responsibilities
* Accountable for care standards and the delivery of care promoting a culture of continuous improvement of clinical standards with particular emphasis on patient experience and key clinical outcomes. Involve patients in the planning of care wherever possible, and act in the best interests of the patients at all times.
* Adopt a case management approach to ensure that rehabilitation reablement is actioned and integration of care takes place.
* Assess, plan, implement and evaluate care and treatment, ensuring the highest quality of patient-centered, goal-directed care at all times, with appropriate involvement and support of relatives and carers.
* Interpret and analyze clinical and non-clinical facts to form an accurate diagnosis and prognosis in a wide range of conditions, to recommend the best course of intervention and to develop comprehensive management strategies for the patients' presenting condition.
* Enhance and develop the skills and dexterity, coordination, and palpatory senses for assessment and manual treatment of patients. Be safe and competent in the use of therapy equipment.
* Refine treatment techniques in line with the most recent evidence of clinical effectiveness by evaluating and implementing key developments and promoting discussion within the service.
* Provide complex information to the patients, clinicians, and other agencies to assist in decision-making, using persuasive, motivational, negotiating, training, empathic and reassurance skills.
* Work with a defined caseload of patients with a broad range of health needs, using knowledge to carry out assessments, using standardized tools, setting objectives, and implementing appropriate treatment as part of the holistic care plan.
* Formulate an individual management/treatment plan for each patient using clinical reasoning skills and utilizing a wide range of physical, sensory, and cognitive treatment skills following analysis of information received from client, carers, referrer, and any other professionals involved in the patients' care as necessary.
* Ensure own and other staff's interventions are evaluated and treatment is modified to measure progress and ensure effectiveness of intervention.
* Provide spontaneous and planned advice, teaching, and instruction to relatives, carers, and other professionals to promote understanding of aims of therapy to ensure a consistent approach to patient care.
* Have working knowledge of equipment, minor adaptations, and resources including activities of daily living equipment, therapy and rehabilitation aids, moving and handling equipment, and a basic knowledge of wheelchair provision in order to compensate for unresolved functional deficits, in line with local resources, guidelines, and legislation.
* Proactively participate in multidisciplinary team meetings.
* Complete clinical documentation and electronic data entry to a high professional standard and in accordance with Trust policies.
* Promote confidence in the patient by means of appropriate professional standards of appearance, behavior, and attitudes.
* Act on unplanned situations using appropriate clinical judgment.
* Safeguard the confidentiality of all information regarding the patient and their family.

Skills and Knowledge
* Awareness and understanding of HCPC and COT standards and other national and local initiatives.
