To support the nurses in developing care plans and resources to support children with delayed toilet training and other continence related issues. Under the direction of the nurses, support families / carers to implement plans and re-evaluate to ensure progression. Meet with Teachers /SENCO to discuss care plan implementation within schools. Establish and develop multi- agency working relationships Liaise with other community and universal services as requested by the nurses Organise and run continence reassessment clinics ensuring that children and young people are receiving the most suitable containment products and promote continence where possible. To participate in the running of toilet training workshops for families. To keep accurate, current records. Participate in the writing of reports and attend / contribute within a variety of meetings as required. Be aware of health related policies and procedures for the school. To contribute to the collation, delivery and presentation of statistical information and data as required by Derbyshire Healthcare Foundation Trust to meet the NHS Agenda. Comply with Trust policies and procedure. A clinical supervision programme will be offered to the post holder.
