We are a knowledgeable, motivated, and supportive team looking for enthusiastic individuals.
Our aim is to ensure that people experiencing a mental health crisis are offered assessment and intervention quickly and effectively.
Responsibilities:
1. Triage urgent/crisis mental health referrals from a variety of sources and establish an appropriate response.
2. Offer mental health assessment, formulation, and time-limited treatment planning for individuals needing an urgent/crisis response.
3. Provide a resource of highly specialized, time-limited interventions to individuals for up to 72 hours following assessment.
4. Work with other disciplines and teams within Sheffield Health and Social Care (SHSC) to ensure service users benefit from a multi-disciplinary approach that best matches clinical need.
5. Develop and maintain close working relationships with existing mental health teams, primary care services, and other key stakeholders.
6. Promote the urgent & Crisis Service through participation in local and national arenas.
7. Work flexibly to develop team working and ensure consistent standards of service delivery throughout wider services.
8. Actively participate in developing team function through tests of change, measuring outcomes, audit, and service evaluation.
9. Work in co-production with service users, providing a high-quality and flexible service to meet individual’s needs.
10. Develop strong links and protocols with other services to ensure smooth and timely transitions for patients moving between services.
11. Include families/carers in decision making and planning.
12. Undertake mental health crisis assessments, formulation, and time-limited treatment planning for individuals needing an urgent/crisis response.
13. Conduct psychosocial mental health and risk assessments in accordance with service policy.
14. Provide comprehensive written reports as required.
15. Participate in clinical supervision both on an individual basis and team/peer supervision, and engage in delivering supervision to more junior members of the team.
16. Maintain clinical records according to professional and Trust standards.
17. Attend and contribute to clinical meetings within SHSC.
18. Work flexibly and provide cover in the absence of other clinicians within urgent & crisis service.
19. Ensure efficient recording of activity and outcome data.
20. Actively take part in and encourage the placement of students within the service.
