We are looking for a GP Care Navigator who is passionate about customer care and giving our patients a positive experience whenever they contact the surgery.
The role will be 28.5 hours over 3 days: Monday, Tuesday, and Friday - working 8.00am/8.30am to 6.00pm/6.30pm, depending on the rota.
Main duties of the job
To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practice's multidisciplinary team. The Care Navigator will also be expected to undertake reception duties as part of the role, supporting the administrative team in delivering a polite and professional service to the entitled patient population.

We offer preventative care such as vaccinations, health screening, and health education. We play a vital role in managing chronic diseases such as diabetes, hypertension, respiratory, and heart disease.
The practice has been based at West Paddock since 1993 and we work as a team-based care including GPs, Advanced Nurse Practitioners, Practice Nurses, Healthcare Assistants, Nurse Associates, Medicines Co-ordinators, Administrators, Care Navigators, and Practice Management.
Worden Medical Centre is part of Leyland Primary Care Network, who provide the Practice with a Paramedic, Pharmacist, and Physiotherapist. We have support from the Lifestyle Improvement Team who provide non-medical social care interventions.
Job responsibilities
The following are the core responsibilities of the Care Navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:
1. Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition.
2. Answer incoming phone calls, transferring calls or dealing with the callers' requests appropriately.
3. Deal with patients who present at the reception desk.
4. Process patient requests for appointments.
5. Initiate contact with and respond to requests from patients, team members, and external agencies.
6. Enter read-code data on EMIS.
7. Photocopy documentation as required.
8. Data entry of new and temporary registrations and relevant patient information as required.
9. Input data into the patients' healthcare records as necessary.
10. Direct requests for information, e.g. SAR, insurance/solicitors letters, and DVLA forms, to the administrative team.
11. Manage all queries as necessary in an efficient manner.
12. Monitor and maintain the reception area and noticeboards.
13. Support all clinical staff with general tasks as requested.
14. Complete opening and closing procedures in accordance with the duty rota.
In addition to the primary responsibilities, the Care Navigator may be requested to:
1. Participate in practice audit as directed by the audit lead.
2. Scan patient-related documentation and attach scanned documents to patients' healthcare records.

Experience
* Experience of working with the public.

* Experience of working in a healthcare setting.

Qualifications
* Educated to GCSE level or equivalent.
* Active signposting or Care Navigator training qualification.
* Healthcare qualification (level 2) or working towards gaining equivalent level.
