The Position:
We are seeking a full-time bilingual Parent Partner for our Palmdale site. This position is primarily responsible for assisting families with locating and accessing social services available in their community.
What You’ll Do:
* Manage and facilitate caregivers’ utilization of the Parent Library. Host Parent Library Tues-Thurs from 3:30 PM – 7:00 PM. Utilize existing educational materials and resources to develop programming, information exchange & supportive atmosphere for Caregivers.
* Accept case assignment from supervisor, reviews client’s record and contacts treating therapist to obtain case history and clarify the case management request.
* Meet with family to complete the CRS psychosocial assessment for the purpose of identifying the family’s strengths and needs.
* Develop a case management treatment plan that will address the concerns of both the family and the treatment team (i.e. treating therapist, psychiatrist).
* Interface with community agencies through phone contact, community meetings and/or site visits to locate organizations that offer services requested by the family, determine that the services are culturally sensitive and adequate to address the family’s unique needs.
* Facilitate any needed skill-building for caregivers and/or clients to support case management goals (including but not limited to teaching effective communication skills, assertiveness skills, organizational skills, independent living skills).
* Complete clinical documentation in a timely manner of all services provided in accordance with the standards set by the State Department of Mental Health.
* Hours: Full-Time (40 hours), Monday: 9:30 AM – 6:00 PM, Tuesday – Thursday: 10:30 AM – 7:00 PM, Friday: 8:30 AM – 5:00 PM.
* The first four months of this position is fully onsite with a potential for hybrid work options after four months of successful employment.
Qualifications:
* Bilingual in English and Spanish required.
* Must continuously have a clean driving record in order to be insurable under the Center’s automobile insurance policy.
* Must be comfortable initiating contact and communication with caregivers, strong interpersonal skills for building relationships and creating a welcoming atmosphere.
* Knowledge of community resources required.
* Must have a good sense of judgment as pertaining to the position.
* Must be able to prioritize and plan work activities and use time effectively.
* Must be a self-starter and be able to work independently.
* Must possess compassionate communication skills.
* Must demonstrate accuracy and thoroughness and look for ways to improve and promote quality.
* Must be able to maintain confidentiality.
* Must be able to work as a team player, in a teamwork environment.
* Must be enthusiastically committed to the overall well-being of the department and the objectives of the Center.
* Experience working with children and families.
* Must be the parent or primary caregiver of a youth or child.
