The team is responsible for upholding the guidance provided by the National Framework for NHS Continuing Health and NHS-funded Nursing Care (Revised 2022) and therefore, comprehensive training is provided to ensure this specialist knowledge is gained. Main duties of the job We are looking for a registered professional who is experienced in working with people who have complex physical and mental health needs. The post holder will support the Learning Disability Team with implementation, supporting and reviewing clients with Personal Health Budgets and undertake the role of Continuing Healthcare Practitioner. NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs. The Complex Cases Team is responsible for the decision making and commissioning processes, working closely with the Local Authorities throughout Cambridgeshire and Peterborough. Successful candidates will have excellent communication and assessment skills; be resilient, able to work autonomously but also work as part of a team. This is a home-based role, and post holders will be expected to be able to travel across Cambridgeshire and Peterborough.
