The Discharge Lounge at Lincoln Hospital is looking for a Nursing Associate to join our multi-disciplinary team which specialises in safe and effective patient discharges.
Discharge Lounge (DL) provides an essential nurse-led service in accepting patients on the day of discharge to release capacity for accommodation of acute admissions. DL supports timely and safe discharge of patients from inpatient wards, assessment units and the Emergency Department (ED), thus improving patient flow through the emergency and planned care pathways by releasing inpatient bed and ED capacity.
DL provides a safe and pleasant environment for up to 18 patients to be cared for whilst waiting for the final elements of their discharge/transfer to be completed. We operate single sex accommodation and are currently open 24 hrs a day, 7 days a week.
Main duties of the job
The nursing team is expected to deliver direct patient care, whilst coordinating the discharge of medically optimized patients. The role requires excellent communication and organisational skills and an ability to prioritise workload. This role is patient-centric; an active interest in patient and family engagement is essential. The ideal candidate will have the ability to compassionately communicate with patients and family, ensuring a smooth transition from hospital to home or other specified destination; answering their queries and allaying or addressing any concerns.
We are able to accept applicants for full and part-time posts. Flexible working arrangements will be considered.

Qualifications
* Registered Nursing Associate on the NMC register
* Nursing Associate Foundation Degree or equivalent
* GCSE Grade A-C in Maths & English or skills level 2 or equivalent
Previous Experience
* Experience of working in teams under appropriate supervision as part of the multi-disciplinary team
* Insight on how to evaluate own strengths and development needs seeking advice where appropriate
* Experience of providing and receiving complex sensitive information
* Awareness and Knowledge of the dignity in care agenda.
* Understanding of the scope of the role of the nursing associate in the context of the nursing and broader healthcare team
* Awareness of how the role can contribute to service development
* Understands and acts in accordance with the NMC professional standards for practice defined within the code
* Knowledge of when to seek advice and escalate to the appropriate professional for expert advice and help
* Understands the importance of following procedures and treatment plans as defined by the Registered Healthcare Professional
* Ability to take part in reflective practice and clinical supervision
* Understand requirements for NMC professional revalidation
* Ability to organise and plan own delegated workload
* Ability to deal with non-routine and unpredictable nature of workload and individual patient contact
* Ability to communicate effectively (written and verbal and non-verbal) with team, patients and their family/carers
* Ability to develop effective and appropriate relationships with people, their families and carers/colleagues
* Ability to support, assess and act as a role model for trainee nursing associates, other learners and healthcare support workers as required within the clinical setting
* Awareness of the need for confidentiality
