Central Cheshire Integrated Care Partnership (CCICP) is expanding its team of General Nursing Assistants (GNAs) who work as part of the Integrated Placement of Care Hub. GNAs provide care and support to patients at home, including assisting with personal care, medication administration, preparing light meals, assistance with catheters or stomas, and support with rehabilitation exercises.
CCICP seeks individuals with experience in care or as a Health Care Assistant, who are passionate about providing high-quality care to patients.
GNAs work across CCICP's five care community hubs: Nantwich, Northwich, Winsford, Crewe & SMASH (Sandbach, Middlewich, Alsager, Scholar Green, and Haslington). Shifts are available from 8am - 4pm and/or 2pm - 10pm, across 7 days a week. Both full and part-time roles are available, with full training provided.

Main duties of the job
As a General Nursing Assistant (GNA), you will work as part of the Integrated Placement of Care Hub (IPOCH), focusing on a 'home first' approach for rehabilitation and promoting independence for patients leaving acute settings or requiring step-up care at home.
A significant part of the role involves supporting end-of-life patients in their homes, providing dignified and quality care for patients and their families during challenging times.
You will work under the direction and support of the wider care community teams, including district nurses, therapy teams, and advanced community practitioners. Full training will be provided by a dedicated GNA trainer, with mentorship from a band 6 nurse for personal appraisals and one-to-one meetings.
Day-to-day support will be provided by senior GNA practitioners dedicated to your development and ensuring you have the skills needed to provide the best care.
The successful candidate will undertake record keeping using an electronic device, with training provided. Applicants must hold a valid UK driving licence.

Detailed job description and main responsibilities
1. Work under the direct supervision and guidance of the nursing team.
2. Act on own initiative in delivering patient care in the community.
3. Support multi-disciplinary health professionals in providing a comprehensive rehabilitation programme to prevent avoidable hospital admissions.
4. Provide care and rehabilitation for service users within their homes under the clinical direction of qualified staff, including assessing patients’ conditions through observation and clinical skills, recording appropriately on EMIS, and ensuring timely communication with relevant professionals.
5. Communicate factual information to patients/carers requiring patience, persuasion, and reassurance.
6. Provide high-quality care for patients and their families within the community setting, including end-of-life care.
7. Handle patients' personal possessions and ensure the safe and proper use of equipment.
8. Maintain accurate personal and professional records adhering to trust policies and procedures.
9. Support patients with the administration of medications.
10. Ensure efficient and cost-effective utilization of all resources (human, physical, and financial).
11. Report accidents/incidents of patients or self occurring while on duty and complete appropriate forms immediately.

Qualifications
* Previous experience of working in the NHS and/or a nursing/caring environment.
