Job summary
An exciting opportunity has arisen for a Senior Community Mental Health Nurse to join our dynamic Older Persons Community Mental Health Team based at Princess Road Clinic in Malton - Ryedale MHSOP CMHT.
As a senior nurse, you will have a genuine interest in collaborative working with older adults experiencing functional and/or organic mental health problems including their carers and families. You will help the team to provide wellbeing and recovery-focused care for individuals with complex and challenging mental health needs, including dementia, depression, anxiety, and psychotic illnesses, within community-based settings.
Applicants must have the ability to travel independently in accordance with Trust policies as the post covers a wide geographical area.
The position advertised is for a band 6 registered nurse; however, we would also welcome applications from band 5 applicants who do not currently meet all the band 6 person specifications as a development post from 5 to band 6. A competency framework will be developed for the successful candidate based on the skills/knowledge gaps of the individual in fully meeting the essential criteria as outlined in the person specification, with a timeframe for completion. Applicants must be clear on which essential criteria is not yet met at application stage.
Main duties of the job
This is an ideal opportunity for someone who wants to develop their leadership and clinical skills in a community setting. As a core member of the team, you will also be responsible for your own caseload, providing assessments, care co-ordination, and delivering intervention in the community with people who have complex needs ranging from mild to moderate level of need to severe and enduring level of need and complexity. You will demonstrate excellent assessment and analysis skills, interpersonal, communication, and team working skills. You will need to be committed to working collaboratively with a range of statutory and third sector agencies to promote holistic wellbeing and recovery-focused care.
Ryedale MHSOP CMHT is a lively, dynamic, and supportive multi-disciplinary team committed to collaboration and joint working. You will support the team's improvement activities for co-creation, aiming to deliver care that is highly personalised to each individual's needs and working closely with families, paid, and unpaid carers.
Applicants need to be able to travel independently for this role, as the team visits people at their homes and other care settings.

The vacancy is within a community-based service that delivers care across a wide geographic footprint associated with two large local Primary Care Networks - North Riding PCN and South Hambleton and Ryedale PCN. The team is committed to providing excellent evidence-based quality care to older adults. It is imperative that the successful candidate has a flexible pro-active approach to their person-centred practice.
Nursing applicants should already hold NMC registration and have successfully completed their preceptorship programme. Please note: Newly qualified Nurses or those due to qualify will be recruited via a separate process.
The successful candidate will be required to successfully complete a competency framework to progress from band 5 to band 6, keeping a portfolio of evidence and clinical achievement to support the uplift to band 6. This is a structured process which will be supported through clinical and management supervision. All staff must have been in a band 5 post for no less than 18 months before they can transition to a band 6 as recommended nationally by Health Education England and NHSE.
