Job summary We are looking for a part time registered nurse to support our PCN Care Home Team. This is a new role created to support the delivery of the Enhanced Health in Care Homes Framework in Primary Care. Working as a member of our PCN Care Home team comprising of clinical and non-clinical roles. To support the provision of a high quality, person-centred approach to care delivery, which always considers peoples safety, privacy and dignity. Delivering care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multidisciplinary care team. Main duties of the job The Care Home Team Nurse will work within their scope of clinical practice and identify residents requiring personalised care and support plans and support the development of these. Ensuring all long term residents and those in short term beds have an up to date escalation plan in place. Signpost, or refer patients at risk of developing long-term conditions, preventing adverse effects on the patients health. Examine, assess and diagnose patients and provide clinical care/management as required. Triage patient and provide the necessary treatment during care home reviews.
