Working 5 days a week over 7 on a rota basis (to include weekend working, max of 2 weekends a month). Flexi time is applicable, and starting and finishing times are flexible. Start date to be confirmed with successful applicant.
An exciting opportunity has arisen within Adult Social Work Services for a Community Care Worker to work within a new prevention and early intervention multi-disciplinary team based at East Lothian Community Hospital within East Lothian Health and Care Partnership. Working in conjunction with our NHS partners and those from Community First, the hospital flow and discharge system, reablement, social work, and care at home, the approach aims to prevent people from entering hospital and also to facilitate a quicker discharge home reducing the negative impact of unscheduled care in hospital.
You will apply an “In Reach” ethos within the hospitals in Edinburgh and also be assessing those in need in the community. Adopting early intervention approaches informed by the new Ageing and Frailty standards and those planned to be rolled out across East Lothian throughout 2025, this will be embracing new ways of working. You will also remain embedded within adult social work services and will have close ties to our adult social work service team based in John Muir House in Haddington.
The post requires working over 7 days, and it is envisaged that weekend working will be a maximum of two weekends per month.
You will be eligible for the council’s flexible working scheme; however, due to the level of agility and responsiveness requirements of the job, set days cannot be guaranteed. Furthermore, on-site working will also be required; therefore, homeworking will be at a minimum.

Job Requirements
Relevant Qualifications
Educated to HNC or SVQ level 3 in social care, or equivalent, and be able to demonstrate equivalent knowledge, skills, and competencies gained through relevant experience.
Driving Licence
As the duties of this role require you to effectively travel to successfully undertake the full remit of the role, you must hold a current driving licence and have access to a vehicle.
