General Practitioner Ystwyth Medical Group
Comprised of three GP partners, four salaried GPs a prescribing Clinical Pharmacist, Pharmacy Technician, Prescribing Advanced Paramedic, Nurse Practitioner and highly professional nursing and administrative support teams. Applications are welcomed from those wishing to join our thriving, multi-disciplinary GMS practice. We would welcome them as a salary GP in the interim, with a commitment to joining the partnership in 2025.
We are very supportive of those with special interests seeking 4 8 sessions per week.

Main duties of the job
Due to a GP emigrating, we are looking to employ for 4-8 sessions per week, over 2 - 5 days, with a commitment to partnership in 2025.
We are a rapidly growing, forward thinking, innovative practice that is part of the North Ceredigion GP Cluster. We value holistic patient care with sustainable workloads, supported by our close PHCT and on site District Nursing Team.

Our family friendly practice is situated in the sea-side town of Aberystwyth, West Wales. Aberystwyth boasts a truly family friendly environment, with excellent schools and further education facilities as well as public sector employers and a well-established university all of which are surrounded by beautiful coastal and mountain terrain offering a plethora of down time activities.
We are a high achieving practice operating from a purpose-built premises and provide high quality care to our 9100 patients, using the INPS Vision clinical system ,EMIS from January 2025.
Job responsibilities
The successful candidate will undertake a variety of duties including surgery face to face, telephone consultations and queries and where applicable visiting patients at home.
Checking and signing repeat prescriptions, dealing with queries, paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self referred or referred from other health care workers/organisations.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols and developing care plans for health.
Providing counselling and health education.
Recording clear and contemporaneous consultation notes to agreed standards and read codes.
Compiling and issuing computer generated acute and repeat prescriptions.
Prescribing in accordance with practice prescribing formulary.
Completion of medical records.
Adhering to Practice polices and procedures.
Ensuring adherence to GDPR and confidentiality.
Adhering to equality and diversity.
GMC with no restrictions.
Currently on the All Wales perfomrers list.
Excellent written and verbal communication skills.
Clear and polite manner.
Effective time management.
An advocate for the Practice.
Evidence of CPD.
Clear enhanced Disclosure and Barring check compulsory.
