Based at Civic Centre with flexible working options, you will support practitioners to work across locality areas reaching out to community-based organisations. You will enjoy supporting consistency of approach in the completion of Mental Capacity Act assessment, Safeguarding Enquires, Care Act assessments and support plans leading to the commissioning of services to meet identified need where non-commissioned support, Technology, equipment, or wider community assets could not ensure independence outcomes.
