An opportunity has arisen for a Social Care Practitioner to join our friendly and supportive Erewash South Adult Social Work Team. The post is part time, 28 hours and 30 minutes per week.
The successful candidate will be based in Erewash with the opportunity for hybrid working. Training and support will be provided which includes regular supervision, peer support, team and area meetings alongside access to Derbyshire County Council training .
You will work in partnership with clients, their families and carers to undertake Care Act (2014) assessments, create support plans and regular reviews. This will be through person centred and strength-based approaches to commission support and signposting promoting the home first culture. The team also supports residents for whom there are safeguarding concerns and works with The Mental Capacity Act (2005).
You will also work with other stake holders for example care homes, housing departments, NHS and many others to support clients through a multi-disciplinary approach.
