We are looking to recruit one full time Consultant Diabetologist to cover Maternity leave for 12 months, to develop our local community diabetes services.
They will be based within Lewisham and Greenwich NHS Trust and spend 50% of their time working in the community across the boroughs of Greenwich and Bexley.
The service will aim to deliver community diabetes clinics, virtual clinics, and primary care MDTs, which involve discussion of complex care patients who have diabetes and other care needs within the multidisciplinary team, with the aim of delivering patient-centred care. The service will work closely with primary care to improve the quality of care across the Bexley and Greenwich system.
There will also be the opportunity to support any additional specialist interest post holders may have within the acute component of the role. The acute hospital-based role can be designed around a post holder's interests to support our existing services in outpatient Type 1 diabetes clinics, pre-conception clinics, gestational diabetes and inpatient diabetes. Alternatively, there is the opportunity to deliver some general medicine work (e.g. acute medicine on-call) or endocrinology clinics.
Main duties of the job
The 10 PA contract will comprise 7.5 direct clinical programmed activities and 2.5 supporting programmed activities. The supporting programmed activities will include 1.5 PA for revalidation including participation in morbidity & mortality reviews, in the departmental clinical governance programme and in mandatory training. It is anticipated that a further 1 SPA will be allocated for teaching and training activities including educational supervision, plus development of the community service.
Job Responsibilities
The Community role is envisaged as follows:
* To provide clinical leadership for a borough based multidisciplinary team including DSNs, podiatrists, dietitian and psychologist.
* To deliver Consultant led community diabetes clinics (type 1 and complex type 2), provide clinical support to the DSNs, podiatrists, dietitian and psychologist.
* Using Practice data, work with Practices to risk stratify their patients with diabetes and deliver MDTs to agree a management plan.
* Offer advice and guidance either by telephone or email.
* To have the opportunity and flexibility to develop this role and service so that it meets the needs of the local community and system partners.
* To test and explore innovation within the service, for example, the use of digital and remote monitoring.
The acute role would be to facilitate continuity and a high quality of diabetes care for patients with established or newly diagnosed type 2 diabetes admitted to the hospital, during their stay and on discharge. We envisage that they will undertake type 2 diabetes and possibly more specialist clinics of their choice. Colleagues will continue to also provide T2DM community clinics to facilitate team working. There will be considerable flexibility within the role to maintain and develop clinical skills.
On Call Commitment
There may be opportunities to undertake inpatient diabetes/endocrine reviews, preoperative assessments, endocrinology clinics or engage in the on call rota depending on the appointee's interests. If applicants wish to join the on-call rota there will be consideration for time off in lieu.
Professional Leave
Must Be Agreed With The CD/DD And Is Discretionary According To The Activity. No Clinics should Be Cancelled For Professional Leave.
Person Specification
Qualifications Essential
* MBBS or equivalent.
* Full registration with the GMC and hold a license to practice.
* MRCP (UK) or equivalent.
* To be on the GMC Specialist Register for diabetes, endocrinology and ideally general medicine or within six months of obtaining CCT at the time of interview.
Clinical Experience Essential
* Broad based training and wide experience of diabetes type 1 type 2 in the hospital and community.
* Depending on the interests of the candidate evidence of experience in specialist diabetes, General/Acute Medicine and Endocrinology.
* Experience of working in a multicultural environment.
IT skills Essential
* Ability to use Microsoft office packages, e.g. MS Word, Excel, Access & PowerPoint.
* Ability to use patient information systems.
