Job summary
An exciting opportunity has arisen for a part-time Band 3 Clinical Support Worker in the Community Diabetes Service. The candidate will be working within a Multidisciplinary team and embedded within a successful, well-established diabetes service. The person will be working closely with both community and secondary care diabetes specialist colleagues to deliver high-quality care to people of Nottingham City living with Type 1 and Type 2 diabetes; this role is a pivotal part of the success of the team.
The successful candidate will be a motivated and enthusiastic individual with proven, well-developed care delivery skills whilst also showing empathy and understanding of the challenges of self-managing a long-term health condition. Experience of working in a caring environment in a hospital or community setting would be essential for this role.
Based at Sherwood Rise Health Centre, but will need to travel to other Nottingham City locations to deliver clinics, we are looking to recruit on a part-time, permanent contract. Working days to be agreed with the successful applicant.
Main duties of the job
The appointed support worker will work using their own initiative with the support of the wider multidisciplinary team.
You will undertake clinics, review foot care, phlebotomy, observations, and provide basic diabetes advice to patients.
You will need to demonstrate sound clinical, interpersonal, and communication skills and must be able to prioritize tasks and manage time effectively.

Job Purpose
To work with the multi-disciplinary diabetes team to support the implementation of person-centred care for people with diabetes in Nottingham City.
To deliver clinics and undertake home visits with the aim of gathering information and carrying out assessments that support specialist team members to conduct consultations based on recently acquired clinical information and test results.
* The post holder will receive regular supervision from an appropriate member of the team.
* To carry out foot checks and venepuncture as part of diabetes annual review (nine care processes).
* To educate clients and their carers, under the guidance of a qualified member of staff in the safe and correct use of blood glucose testing equipment/continuous glucose monitoring sensor.
* To work in a variety of settings that may include clients' own homes and care homes.
* To ensure effective communication and exchange of information with clients/carers through direct contact, written communication, and telephone contact.
* To liaise with multi-professional teams and other agencies to promote good outcomes for clients.
* To adapt the approach used to take account of clients/carers' individual circumstances and to feedback to relevant clinician. These may involve complex, unpredictable, and difficult situations e.g. challenging behaviours, family dynamics, or mental health needs.
* To feedback any concerns relating to the needs of patients/carers to the relevant clinical staff.
* To write and maintain accurate and comprehensive records in line with City Care Policy and procedures.
* To comply with health and safety requirements and to maintain and adhere to the organization's policies and procedures.
* To undertake administrative, clerical, and housekeeping duties to promote good practice and organization within the team.
* To ensure effective use of time and organizational skills to maintain a range of activities; this may require flexibility at short notice.
* To maintain appropriate statistical returns in line with CityCare and Legal requirements. This will involve data entry and storage in paper and/or computer format.
* To actively contribute to the promotion and marketing of the service.
Dimensions
* This post has no budgetary responsibility; however, the post holder is responsible for demonstrating cost consciousness.
* You will be required to work across different bases and community settings which may include clients' own home, residential or nursing homes.
* Work from a Systm0ne ledger planning clinic workload up to 3 months in advance with support from the service coordinator.
* You will be required to complete all essential training in accordance with CityCare policy and procedures plus any other identified training.
Key Responsibilities
Clinical
* To undertake annual review assessments based on a defined Year of Care programme template:
o Independently in consultation with a senior member of the team.
o Alongside a clinician when necessary.
* To use communication skills sensitively to gather information on lifestyle choices.
* To use effective communication skills and gain co-operation with treatment where there may be significant barriers to understanding or ability to co-operate with treatment.
* To discuss case management with other clinicians and provide advice and support to members of the team where appropriate.
* To communicate effectively with other clinicians and teams to enable smooth patient journey within the system.
* To establish and maintain effective channels of communication throughout the integrated team.
* To liaise with relevant secondary and primary care staff in order to ensure that the holistic needs of the service users are met.
* To be based within the community whilst maintaining close links and regular communication with the secondary care team.
* To plan and prioritize own workload.

* To attend organized training sessions and complete all relevant competences within agreed time frames.
* To be involved in appraisal systems, clinical supervision, and be active in identifying aims and objectives for personal development in line with City care Policy.

* To keep up to date in basic IT skills in line with the data collection records.

* Experience of working in a health or social care setting. This could include substantial voluntary work or work in a related field e.g. housing.
