Exciting opportunity for a Qualified Social Worker in the Adults Mental Health Team based in Bishop Auckland, earning £202.17 per day umbrella rate. What you will do:
1. Conduct comprehensive assessments of individuals' needs and encourage full participation from users and carers to ensure their viewpoints are integral to the planning process.
2. Develop detailed care plans by designing tailored packages that meet each user's assessed needs. Involve users and carers in discussions about risks and financial contributions towards service costs.
3. Implement these care plans by negotiating services at rates considerate of budget constraints, ensuring they adhere to service principles and quality standards.
4. Lead the monitoring process, ensuring service delivery aligns with both budgetary and qualitative benchmarks, providing continual support to users and carers.
5. Regularly review care plans to ensure they continue to meet objectives, adapting to any changes in needs, while maintaining service value.
6. Maintain high service standards, reporting any deficiencies and working collaboratively to evolve services to better meet user needs.
Qualifications:
1. Degree in SW, Dip SW, CSS or CQSW or equivalent.
2. Social Work England registration.
