What will a day in the life of a Support Worker involve? Providing support to service users (telephone calls, welfare checks, shopping etc), perhaps following a hospital stay or to enable early discharge. Responding to referrals from Clinicians and Community Health Professionals Assess service users’ needs, completing and following a care plan after hospital discharge or to prevent readmission. To be a successful Support Worker, what will you need? Do you have a manual UK driving licence, held for minimum of two years? Are you kind, caring and empathetic with an ability to relate to and help vulnerable people? Do you have a basic IT knowledge? There is no personal care or administration of medication required for this role. Closing date for your application is 5th November 2024.
