Job summary We are recruiting to a Community Team Lead post within our Access and Assessment Team for a 6 month secondment / fixed term contract. The AAT team is our access and assessment services in Suffolk for referrals to Adult Mental health and wellbeing services. The service works closely with the Primary Care Network practitioners and NSFT Mental health services to ensure a smooth transition from referral to assessment and treatment. The role will include line management and operational support to the AAT team, providing clinical support and oversight with MDTs, and complex cases. The AAT service is under transformation, so this is an exciting time to join the team and develop your skills in transformation and operational change to progress the service going forward. You will be part of a multidisciplinary team of professionals working along side the PCNs and community teams. In this role you will provide operational leadership to the team, working closely with our Community Matron and providing clinical support for the team as needed. The Community Team Manager will collaborate with key partners and stakeholders across health, social care and other sectors to support and drive service development ensuring that key performance targets and national standards are met. Main duties of the job This is an opportunity to work within a specialist multi-disciplinary setting, which offers a wide range of MDT clinical assessments and treatments, specific specialist perinatal therapeutic individual and group interventions.
