An opportunity to join an award winning team supporting people living with dementia and their carers and families in Dudley. Hours: 37 per week, working 5 days over 7 Location: The main base for dementia advisors is The Crystal Gateway Dementia Centre in Amblecote, but as an Agile worker you will work mainly from home or in the community. The post holder must be able to drive, hold a valid full driving licence and have the use of a vehicle. T he role of the Dementia Advisor is to improve information and support to people living with dementia and those people newly diagnosed. This is provided both individually and in group sessions to enable peer support within the Dementia Community The emphasis is on supporting the person to live well with their dementia either at home or in their own local community, We do this by providing advice and information, signposting to relevant support and community services and by promotion social Inclusion. This is both for the person with dementia and their supporters, including the immediate carers, families and friends of the people who are referred to the service. There is a focus on prevention and remaining well to help promote wellbeing and also to help delay and prevent the need for more formal services. Experience of working in a community care setting with an understanding of the needs of people with dementia and their carers is essential. Knowledge of relevant legislation including the Care Act, the Mental Capacity Act, Deprivation of Liberty Safeguards and Health and Safety is also required. The role will include participating in a Duty Rota as well as undertaking Social Care assessments to arrange a personal budget for attendance at The Crystal Gateway Dementia centre or Community Outreach service. The ability to produce documents and reports to evidence personalisation using IT systems is required. You will work flexibly to meet the needs of people including weekend where this is required. It is important to have an ongoing commitment to support the continuing development of the Dementia Gateway service as we respond to the needs of the Dementia Community. The successful candidate will be well organised and self- motivated with excellent communication skills. Qualified to NVQ Level 3 in Dementia or an equivalent Health or Social Care qualification or a willingness to undertake is required. You must be willing to undertake training as required for the role, some of which may be in your own time.
