Job summary An exciting opportunity has arisen for a nurse /AHP consultant practitioner with a passion for frailty. We are looking for a dynamic, strategic thinker with experience of working in Frailty to provide leadership, clinical expertise, in the development of our Same Day Emergency Care and expansion of the existing ambulatory pathways. You will become part of our expanding, exciting and outward looking team and will have the opportunity to work together with the multi-disciplinary team to make a difference in the care of our Frailty patients. The successful candidate will take the lead in driving the ambitions for the Frailty service, as well as developing links with partners to allow direct admissions from the community to the frailty service and developing our established ambulatory service. The suitable candidate would have experience of working within a Frailty or Geriatric service and have either or both community or acute hospital experience. The suitable post holder with have experience of leading change, audit, research and education. We are looking for a consultant practitioner with a passion for frailty. This is full-time, 8b post and we are looking for someone to provide leadership in the development of the 7-day service of the Frailty team.
