Does this sound like something you could be interested in? Let's talk about what opportunities are available to match your skills and experience!, Working in a busy and dynamic Adult Community Mental Health Team means every day will be different. You will be involved in:
* Managing a (capped) caseload with a mix of service users, from those under The Care Programme Approach and others that are not
* Undertaking joint initial urgent/routine assessments on behalf of the service
* Attending multi-disciplinary team meetings with our Consultant Psychiatrists, Nurse Consultants, Clinical Team Managers, Senior Practitioners, Assistant Practitioners and Allied Health Professionals including Consultant Clinical Psychologists, Clinical Associate Psychologists and Assistant Psychologists
* Attending outpatient clinic appointments
* Undertaking face to face visits in the community setting including home visits
* Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
* Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management
* Working with other teams services like; inpatient, Home Treatment Team, Psychiatric Liaison Team, Social Care, The Police, Accommodation and Support Teams and 3rd party sector.
* We work with service users on their presenting needs and have many evidence based intervention packages to enhance the Service User experience.
* Use of own car and ability to travel independently is essential
