Key working Relationships
Act at all times in accordance with the NMC Code of Professional Conduct.
Ensure all nursing records are completed in accordance with Southend Hospital policies and NMC standards for records and record keeping.
Main Duties
Make a thorough assessment of the patient’s needs and document these in accordance with Trust policies.
Formulate a plan of care based on the identified needs of the patient, taking into account current evidence based research findings.
Ensure that all the care planned is delivered to the patient at the highest possible standard. Evaluate and document nursing actions and alter the care plan in accordance with the changing needs of the patient.

To act as an advocate for the patient at all times and promote patient health through education
Pro-actively communicate and liaise with all members of the multidisciplinary team on matters
affecting patient care.
Ensure the rights of patients are respected at all times and assist in the provision of information,
enabling informed decisions to be made about their care.
To commence discharge planning on admission, co-ordinate through to the safe discharge of
patients from hospital, as per Southend University Hospital NHS Trust Discharge Policy.
In conjunction with the Ward Manager/Deputy Ward Manager, identify and plan appropriate
interventions that contribute to high staff morale and good working relationships with all members
of the multidisciplinary team.
Positively contribute to the process of change within the ward.
Contribute to the reduction of healthcare acquired infection by ensuring a high standard of infection
control practice in the light of research, current best evidence and legislation.
Adhere to the Trust Policies and Guidelines for the Safe Handling, Administration, Storage and
Custody of Medicinal Products.
