Job summary
An exciting opportunity has arisen for a Band 4 Health and Social Care Assessor within the Bee at Home team. This role is primarily based at The Orchard Centre, where you will join a team of Health & Social Care Assessors to triage referrals into the service, carry out patient assessments in their own home and coordinate the shifts, acting as a point of contact to the Health Care Assistants.
Main duties of the job
Bee at Home is a 7-day week service, operating between the hours of 8am - 10pm, covering Hull and the East Riding. You will be required to cover 3 long days and 1 short shift.
Assessment experience is advantageous.

Person Specification
Qualifications Essential
* Level 4 in Health and Social Care or equivalent level of knowledge, skills and experience
* Care Certificate or willingness to undertake this within induction period
Knowledge Essential
* Knowledge of planning and completing initial client assessments
* Knowledge of policies, procedures and practices relating to domiciliary care
* Knowledge of a range of software programmes including Microsoft Word, Excel and Outlook
* Knowledge of completing risk assessments appropriate to domiciliary care
Desirable
* Knowledge of relevant legislation e.g. Mental Health Act (1983), Mental Capacity Act (2005), Safeguarding Adults and Deprivation of Liberty (DOLS)
* Clear understanding of domiciliary care and client needs in the community
* Knowledge of databases used within domiciliary care
Experience Essential
* Experience of completing initial client assessments either in the community or within a residential setting
* Experience of working in a health and/or social care setting
* Experience of undertaking risk assessments
* Experience of keeping accurate records and inputting data into relevant database systems
Desirable
* Experience of undertaking a similar role within domiciliary care services
* Experience of working with people with a range of health conditions such as dementia
* Experience of working with people with end of life needs
* Experience of liaising and working with multidisciplinary teams or care teams
