Many of those accessing this service may have vulnerabilities or be experiencing hardship. They may have multiple complex support needs which can range from poor mental health and wellbeing, substance misuse, poverty, unemployment and domestic abuse in addition to being homeless. The role holder will be empathetic, practical and outcome focused in leading a team of staff in delivering a holistic, strength-based service to all households. The role holder will lead the vision for the service ensuring a collaborative and coproduction approach is taken in developing the service offer. In the spirit of coproduction, they will include the opinion of the residents, the staff team, and those with lived experience of homelessness in development of the service delivery model. The role holder must have a working knowledge and significant understanding of homelessness, an understanding of the prevention of homelessness agenda and the implications for the individual and the city in this regard. They must have a detailed understanding of safeguarding for both children and adults, and the need to put the child at the centre of support planning for the homeless household. They will understand performance management well and will set targets against which they will monitor and manage the success of their team taking corrective actions with the team where appropriate. Practical activity sessions for individuals afford a key time during which informal engagement and confidence building can take place. The role holder will ensure that a programme of meaningful activity is developed at their accommodation sites to enable stronger engagement work with residents, re-build confidence and support people more effectively to regain their independence and move on. The role holder will forge partnerships across the city to help households to effectively regain their independence, improve choice of accommodation and enable move on for all with greater resilience. The role holder will demonstrate excellent partnership working skills as they build and develop close working relationships with key stakeholders in the Homeless Partnership in the city, the voluntary sector, health partners, Children's Services, Adult Social Care colleagues, organisations and colleagues within the wider Homelessness Service. Being an effective relationship manager, the role holder will work collaboratively to develop the service ensuring it is responsive to need and provides the best wrap around holistic supportive offer to all. The role holder will work closely with the Private Rented Sector team to help move families and singles out of homelessness and into long-term accommodation as quickly as possible to ensure their time as homeless is as brief as possible. The role holder will advocate for the service and participate and represent the service within Homeless Charter groups and in other forums as and when required. The role holder will ensure the provision of a high quality, people-centered service which understands the causes of service user needs in order to improve their opportunities and independence. The role holder will carry out or manage the delivery of complex assessments which responsive to the social care needs of the service user in a sensitive and appropriate manner considering and implementing a range of available solutions and interventions. The role holder will establish and maintain partnerships with a range of stakeholders in line with statutory functions and relevant legislation and Council policy and procedures. The role holder will need to have an understanding of health and safety, fire risk assessment, building risk assessments and building management. Identify, understand and respond to the health and social care needs of service users by providing relevant services to ensure positive outcomes whilst ensuring quality and cost effective services. Deliver professional and detailed advice and assistance with regards to options available for the care and support needs of service users to promote better outcomes. Identify current and potential safeguarding risks and refer where appropriate, assisting in investigations of allegations to ensure vulnerable service users are protected. Provide high quality support to the service management as required, ensuring the efficient and effective deployment and performance of the team in relation to workload and capacity demands. Ensure all records, processes and systems are up to date and maintained to assist with data collection and performance management. Assist with the monitoring and review of service delivery to ensure that performance targets are met and developed, implement and review procedures to continually enhance the service.
