We are looking to recruit a qualified Social worker to join our Prison and Adult Social Work Team to work across the North of South Derbyshire and the Prisons .
We work with adults who have care and support needs within the local prisons ,
adults living in the community and young adults approaching adulthood to complete person centred assessments under The Care Act 2014. The successful applicant will be able to demonstrate the necessary skills, knowledge and experience to undertake this role to provide a safe, accountable, cost effective and efficient service to our client groups to enable them to achieve their outcomes. Key responsibilities will include carrying out assessments under The Care Act 2014 and reviews, as well as arranging the provision of services to meet eligible needs. It is expected that you will be committed to working in a person centred and strengths based way with clients and their families to maintain and maximise their independence .
We undertake personalised safeguarding enquiries where strong communication and organisational skills will be required. An understanding and experience of safeguarding is essential as well as undertaking assessments in accordance with The Mental Capacity Act 2005.
We work alongside other departments and agencies to promote multi disciplinary practice and expect the same of our colleagues .
There is also an expectation that you will undertake the duty role on a rota basis. The role is hybrid working with the ability to travel being an essential requirement .
