Registered Nurse (P/T) - PCN Care Home Team
We are looking for a part-time registered nurse to support our PCN Care Home Team. This is a new role created to support the delivery of the Enhanced Health in Care Homes Framework in Primary Care. Working as a member of our PCN Care Home team comprising clinical and non-clinical roles, you will support the provision of a high-quality, person-centred approach to care delivery, which always considers people's safety, privacy, and dignity. Delivering care and interventions, assessments, treatments, and managing patients on a caseload, as part of the wider multidisciplinary care team.
Main duties of the job
The Care Home Team Nurse will work within their scope of clinical practice and identify residents requiring personalised care and support plans and support the development of these. Ensuring all long-term residents and those in short-term beds have an up-to-date escalation plan in place.
Signpost or refer patients at risk of developing long-term conditions, preventing adverse effects on the patients' health.
Examine, assess, and diagnose patients and provide clinical care/management as required.
Triage patients and provide the necessary treatment during care home reviews.
Job responsibilities

Providing advice to others regarding the management and care of patients/service users.
Devise programs of care (Personalised Care and Support Plans) for care home staff to deliver, monitor, and review to ensure they are delivered appropriately.
Support the planning, implementation, and review of health improvement programmes within the care home environment.
Support the proactive identification, monitoring, provision, and coordinating of care for the PCN's Care Home residents in accordance with the Care Home Standard Operating Procedure.
Person Specification
* Effective communicator at all levels
* Good knowledge and understanding of Primary Care, Care Homes, and the Local System.
* Knowledge of QOF and Enhanced Services.
Qualifications
* Registered Nurse (with at least 1 year of experience)
* Evidence of continued professional development
* Willingness to work towards further educational qualifications required for the role.
* Evidence of working in Care Homes.
Experience
* Experience of working in a team
* Experience of dealing with vulnerable patients
