An exciting opportunity has arisen for a motivated and compassionate registered Children's or Learning Disability Nurse to join All-age Continuing Care, a team driven to transform current practice and make positive changes to children, adults, and their families.
An experienced and highly motivated Registered Nurse with an excellent knowledge base of complex health needs is sought to work as a member of the All-age Continuing Care Team, working to improve health care outcomes in Lincolnshire.

Main Duties of the Job
We are seeking a Registered Nurse who has excellent clinical knowledge as well as communication and assessment skills. The post holder needs to be proactive, resilient, and able to work autonomously as well as part of the wider All-age Continuing Care Team. They will ensure that health care needs of both children and adults eligible for NHS Continuing Care or NHS Continuing Health Care are identified and met in accordance with the National Frameworks. They will have an opportunity to build cross-agency relationships while working as a multidisciplinary team with colleagues from external agencies. This community-based position offers a Registered Nurse the opportunity to enhance assessment and decision-making skills in a challenging but supportive environment.
The case manager will carry out assessments of need, reviews, and attend statutory meetings for individuals with life-limiting conditions and complex health needs. They will ensure the needs of adults with learning disabilities are recognized and supported under the Section 75 agreement of the NHS Act 2006. Keeping the relevant National Framework and NMC Code of Professional Conduct at the forefront of decision-making, they will work within established policies, procedures, and protocols ensuring quality standards and performance as directed by the Children's Clinical Lead and Head of All-age Continuing Healthcare Clinical Services. In addition, they will be expected to provide support across Lincolnshire ICB.

Job Responsibilities
* Supporting the clinical lead nurse and head of Continuing Healthcare in the development of a range of services for children and adults with complex health needs, including learning disabilities and Autism.
* Work as an autonomous nurse within a multi-agency setting.
* Collating and undertaking comprehensive and coordinated assessments for decision-making for eligibility for All-age Continuing Care as part of a multidisciplinary team with families and agencies from health and social care.
* Undertaking specialist assessments for children and adults to determine packages.
* Supporting young people and their families through transition to adult services for their package of care.
* Supporting the clinical lead and head of service to develop transition pathways.
* Validating comprehensive and coordinated assessments completed by external agencies for adults with learning disabilities in relation to the Section 75 agreement of the NHS Act 2006.
* Representing All-age Continuing Care in interagency disputes for adults and children with learning disabilities, by attending relevant panel meetings.
* Promoting Personal Health Budgets and assisting individuals to utilize their Personal Health Budget in accordance with national guidelines.
* Providing professional nursing advice to acute hospital trusts, residential homes, and social services regarding All-age Continuing Care and Personal Health Budgets.
* Support with All-age continuing care training.
* Case managing All-age continuing care packages, ensuring timely reviews are conducted and ensuring care packages continue to be effectively commissioned and are cost-effective for the NHS.
* Assisting in maintaining a database for all individuals assessed for All-age continuing care.
* Developing strong working relationships with partner agencies including acute hospital trusts, local authorities, the independent sector, and others.
* Act as an advocate for patients and carers who are identified in the complex and vulnerable client group in partnership with local authorities and healthcare providers to ensure needs are met in the appropriate setting.
Person Specification
Skills and Knowledge
* Active UK Nursing registration (child or learning disabilities).
* Evidence of continued professional development.
* Knowledge of adult and child safeguarding processes and responsibilities.
* Aware of Caldicott principle and information governance.
* Understanding of Confidentiality and Data Protection Act.
* Awareness of equality and valuing diversity principles.
* Demonstrated capabilities to manage own workload and make informed decisions in the absence of required information, working to tight and often changing time scales.
* Clinical knowledge base to ensure comprehensive holistic assessment for All-age Continuing Care.
* Able to demonstrate strong knowledge base around safeguarding adults, children, and consent.
* Good clinical background to show knowledge base to ensure scrutiny of compliance with statutory guidance and best practice.
* Good clinical background to understand complex health needs.
* Negotiation and conflict management skills and the ability to influence in formal settings.
* Demonstrated capability to plan over short, medium, and long-term timeframes and adjust plans and resource requirements accordingly.
* Application demonstrating understanding and an interest in CYP transition.
* Knowledge and understanding of the national policies and guidance relevant to the National Framework for Continuing Care and Continuing healthcare.
* Expert knowledge of All-age Continuing Care pathways including screening, assessment and decision-making processes, review, and current appeals.
* Good knowledge of NHS and Social Services guidance, Continuing Care, and Continuing healthcare national frameworks.
* Good knowledge base around CYP transition and pathways of funding for learning disabilities.
* Demonstrates a keen desire/passion to make positive changes to CYP outcomes.
