1. PURPOSE OF JOB The primary purpose of this post is, as part of the management team of the Community Resource Centre, is the efficient and effective provision of person-centred services for older people, designed to meet assessed needs of service users including those living with dementia, physical and learning disabilities, and sensory impairments. To enable individuals to live as independently as possible in their care setting. This will involve providing services to encourage service user functional rehabilitation. 2. PRINCIPAL ACCOUNTABILITIES Service Delivery Departmental With the Registered Manager participate in the formulation of detailed team objectives and policies. With the Registered Manager ensure the effective and efficient implementation of Council policies and the achievement of the Council's objectives, including financial objectives. To contribute to co-operative working across services in accordance with the Council's Vision and Values. To contribute to cross-service initiatives as required. To assist in ensuring the Team's services are responsive to community needs and that equal opportunity and health and safety issues are identified and addressed effectively. To assist in ensuring effective external and internal working relationships are established and maintained with organisations and agencies relevant to the work of the service. To ensure that effective internal working relationships are established and maintained with staff in the Residential Service and their trade union representatives; and between the Residential Service and other departments. To ensure effective and accessible communication with staff, service users, the general public and others as appropriate. b) Functional Service-user care To coordinate the provision of holistic care and support of the care floor to meet users physical and psychological needs and to promote the continuing spiritual, family and social life of the residents both within and outside the home helping them to maintain social interaction with family, friends and the community in line with their wishes. To coordinate the delivery of personal care to individuals, following instructions and recognised training received. This may include, for example, help with: Getting up/ putting to bed Personal hygiene Dressing /undressing Use of toilet/commode Catheter care including stoma and sheath care Assisting with prostheses (false legs/eyes etc) Application of simple dressings Using sterile techniques Foot and hand care Basic nursing care during illness and terminal illness Health promotion Person-centred support for those living with dementia To assist the Registered Manager in monitoring the effectiveness of the key worker systems within the home, including the preparation, implementation and review of individual care plans, and attending reviews. To liaise with medical and nursing practitioners, medical establishments, social workers and other agencies on all relevant matters, including the admission, discharge, transfer and death of service users. To assess the progress of service users and to maximise their independence levels, encouraging them to achieve their full potential. To monitor the general health and well-being of service users for any signs of deterioration, so that early intervention can ensure that preventable measures are taken to avoid unnecessary hospital admissions. To be familiar with and practice in accordance with the Councils Safeguarding Adults Policy and practice guidelines and always relate any issues or concerns to a line manager. To maintain accurate service user records, e.g. Care Plans, as required and in accordance with Council Policy and good practice. To promote effective communication and positive relationships, and, where there are communication difficulties, to enable service users to have access to advocates where appropriate. To operate and promote an effective and responsive complaints procedure in line with Council policy. To ensure the proper control, recording and issuing of medications as prescribed by a medical practitioner and in accordance with council policy. To support those who are able to administer their own medication. To ensure the residential services are compliant with the guidelines and standards set out by the Care Quality Commission (CQC) and other statutory and legal frameworks and to respond appropriately to inspection reports. Care of the Environment To maintain a high standard of hygiene and cleanliness throughout the establishment, ensuring that the appropriate documentation is completed in accordance with legislation and Health & Safety guidelines. To ensure that adequate supplies and provisions are requisitioned, recorded and used appropriately. To ensure that the fabric, decoration and equipment is maintained to a satisfactory standard in line with Health and Safety requirements, including referral to appropriate agencies when necessary. Catering Assist the Registered Manager in ensuring that the catering staff provide a high standard catering service according to service user preferences and dietary needs, and the requirements of Food Hygiene regulations. Working in a team Regular contact is to be maintained with all service users and their carers as appropriate), to ensure that care plans are being carried out effectively. Include working with Social Workers, GPs, Community Nursing staff and other health/ housing professionals and other agencies e.g. Age UK. Access to a Deputy Manager or Registered Manager on a regular basis for supervision and guidance. Management To assist in managing and supporting the staff team in the performance of their day to day duties including the allocation of work rotas, supervision and support. Adverse Event reporting: to complete the notification of adverse events in line with the local authoritys governance requirements. To identify poor staff performance and report to the Registered Manager so that any action can be initiated. To undertake the supervision of a small group of staff and develop awareness and skills through one-to-one and group meetings and identification of training needs. To support and assess progress of those undergoing qualification/training eg Care Certificates, to ensure quality and completion of the programme To occasionally assist with the recruitment and selection of staff. To assist with staff training and practice improvement of individuals in conjunction with other senior colleagues to participate in the induction programme for new staff. To promote and maintain the quality assurance monitoring systems and observe the Councils principles of Best Value. To be responsible for the general supervision of the Home in the absence of the Registered Manager or Deputy Manager. Service Development and / or Delivery Work very closely with others in the Council to share information and ideas, and promote a consistent, organisational-wide approach to policy and practice. Support the ongoing review and development of any policies procedures and processes. Keep up to date with external developments in practice to inform changes in policy and procedure and for use as appropriate when advising line managers. Work closely with others in the Council to promote, monitor and maintain health, safety and security in the workplace. To follow the requirements of the Health and Safety at Work Act, Food Hygiene Regulations, and to be responsible for the safe use/storage of cleaning materials in accordance with COSHH regulations. Work within the requirements of be involved in the completion of risk assessments and be responsible for accurately reporting any situations which could be considered hazardous or a safeguarding concern to the appropriate person/ line manager Participate in the development of the service and support its effective communication and implementation in service areas. Other Adopt a proactive approach to developing and maintaining excellent relationships and communication channels with a wide range of contacts including managers, external clients and suppliers, staff and employee representatives. Represent the Registered Manager / Deputy Manager as required, including attendance at internal and external meetings. Participate in project activity and lead on specific projects or pieces of work on behalf of the Registered Manager / Deputy Manager, taking responsibility for the achievement of outcomes within required timescales. For example, arranging shift cover if the Registered Manager is not available. Or if linked to Personal Professional Development, do a project such as lead on care planning; fund-raising; finance tasks. To undergo training in both formal/informal courses and on-the-job (including undertaking relevant apprenticeship standard training, when appropriate) as identified by Line Manager. DIMENSIONS The Senior Support Worker will work within a designated service portfolio with a high degree of diversity with variable workforce compositions, business needs and priorities. PHYSICAL EFFORT AND WORKING ENVIRONMENT On occasions the postholder will have to provide personal care for a resident and provide emotional support for residents which could mean a combination of noise and odour. 6. GENERAL To be part of a care support team which provides a flexible service, 24 hours a day, 7 days a week including bank holidays. The working arrangements will include evenings, weekends, nights and bank holidays on a rostered basis across both/all floors of the home as required by the Registered Manager Undertake such other duties and responsibilities as are specified by the Head of Service and are commensurate with the level of the post. In an emergency or extenuating circumstances, the post holder may be required to assist in providing or ensuring additional out of hours cover. The team structures within the service operate with a high degree of collaboration and flexibility, individual service portfolios may therefore change over time and post holders are expected to undertake any other duties commensurate with the general level of responsibility of the post. PERSON SPECIFICATION QUALIFICATIONS: To be compliant with Mandatory Training Requirements. Must hold Care Certificate Qualification and Level 3 Diploma in Care qualification (or equivalent) and have received updated mandatory training in relevant areas. Experience in the care of older people and care of those with dementia is essential. A commitment to and responsibility for personal and professional development and actively to take advantage of upskilling through relevant apprenticeship standards and training opportunities for both yourself and others To have had previous managerial experience in residential care of the elderly would be an advantage.
