Job summary Maternity Cover for qualified Nursing Associate with Primary Care/General Practice experience - cover is for 6-9 months. Primary Care based working with General Practices in a Primary Care Network (PCN) and Local Care Partners working in the health Population Management Hub. The post holder will mainly focus on supporting patients with Learning Disabilities and Mental Health. This exciting role with see the Nursing Associates actively contribute to a patients health and wellbeing through the delivery of holistic care, a whole-person approach which considers, and equally values, physical, psychological, public health needs, social economic, spiritual and other factors in the assessing, planning and delivery of care. Providing all relevant competencies (to be developed in line with the requirements of the service) have been achieved, she/he/they will have delegated responsibility for a pre-agreed caseload under the supervision of a nominated clinician within the PCN multidisciplinary. About us The post holder will be integral to the team of Health Population Management Hub of Nursing Associates, Mental Health TNA, care coordinators, the team closely with PCN clinical pharmacists, specialist GPs, Nurses, Health and Wellbeing Coaches, Social prescribers and the wider local care partners like: Community services, hospitals and third sector partners. Working in BHR PCN How you will tackle health inequalities in BHR PCN, caring for patients within the population, the role holder with need to have knowledge and skills to support other common associated health conditions as well as Learning Disabilities and Mental Health. The role will also include supporting and raising awareness of cancer screening for those that are eligible and providing community NHS Health Checks.
