Job summary
Williton Community Hospital are seeking an ACP who can demonstrate experience of all four pillars - 80% in clinical practice, whilst also facilitating learning, leadership and evidence, and research and development.
If you are interested and fulfil the criteria but do not hold the necessary ACP qualifications, we are keen to support the successful applicant to achieve this whilst working within the post at a Developmental Band 7 level.
Main duties of the job
To be responsible for the rapid assessment, diagnostics, and treatment of individuals within Meadow Ward at Williton Hospital using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision-making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.
To plan and manage complete episodes of care. This will be working in collaboration with senior medical colleagues and others when required. Delegating and referring as appropriate, to ensure timely, effective management to optimise health outcomes, in line with the evidence base and nationally recognised best practice.
To play a key operational role and maintain a pro-active approach to managing, updating and leading clinical pathways.
To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence-base, in accordance with values-based care.

Job responsibilities
The post holder will be a skilled, autonomous practitioner working as a core member of the multidisciplinary team to deliver high quality care to patients. They will possess the skills, competence, expertise and clinical decision-making abilities required to plan, co-ordinate and prescribe treatment for older persons mental health patients within the community setting.
The post holder will provide clinical leadership for the development of integrated patient pathways in the Neighbourhood for community nursing which are in line with recognised best practice and evidence base, working closely with stakeholders and care providers.

Qualifications
Essential
* Professional Registration with either NMC, GPhC or HCPC.
* Evidence of MSc level education. Such as:
* MSc Advanced Practice.
* Equivalent / other MSc mapped to the four pillars of advanced practice.
* Demonstrated competence at an advanced practice level through Royal College or NHSE Credentialing.
* Evidence of Continuing Professional Development
* Non-Medical Prescribing (if appropriate to registered profession / role.)
* Leadership/management course or validated portfolio evidence (if not part of MSc level qualification)
Experience
Essential
* Significant experience of working as a registered practitioner in a provider organisation (e.g. acute sector, community provider, third sector, social care or primary care) and within a relevant specialty (suggested 4-5 years minimum)
* Evidence of recent teaching, mentorship and supervision of staff
* Evidence of recent audit and/or research experience
* Evidence of involvement in change management and/or clinical service improvement.
* Experience of multi-professional working

* Evidence of a good standard of literacy/English language skills
* Effective written and verbal communication skills
* Ability to communicate effectively with all stakeholders including to service users, clinicians, and senior leadership teams. This will require proven ability and leadership skills demonstrating motivational, visionary, diplomacy, and negotiation skills.
* Evidence of theoretical and practical skills allowing autonomous patient management (i.e. patient assessment, diagnosis and management) related to clinical scope of practice
* Understand, encourage and support clinical supervision and mentorship both within and across professional roles.
* Ability to interpret and evaluate national and international guidelines and apply to local protocols and practice as appropriate.
* Educational skills such as identification of learning needs, delivery and evaluation, providing work-based learning.
* Works under own initiative with strong organisational skills and ability to achieve to deadlines.
* Effective time-management and prioritisation skills
* Willingness to use technology to improve standards of care and support to our patients.
* Appropriate level of I.T literacy (i.e., for radiology, pathology, microbiology ordering as per Trust protocols, utilising patient systems)
