Oldham Central Primary Care Network are looking to recruit a full-time Cancer Care Coordinator to work across the PCN; the role will be utilized in the prevention, screening and early diagnoses part of the cancer pathway.
This post will particularly be supporting the early cancer diagnosis and cancer care quality improvement work by supporting practices to improve their processes, achieve their targets and working with other services to help ensure patients have the right support at each stage of their journey.
Plans and progress will be supported by meetings with the Greater Manchester Cancer Alliance Early Diagnosis and Primary Care Facilitator and PCN Cancer lead GP.
Main duties of the job
1. Promoting prevention through holding education events.
2. Sourcing, creating, and distributing posters and leaflets across the PCN footprint.
3. Consulting with Digital Champions within practices on advertising preventive advice within practices' social media and websites.
4. Working alongside local community groups to provide patient information, awareness and support to enable proactive engagement/understanding.
5. Identify at-risk populations through the QOF register.
6. Develop and embed systems across the network to improve cancer screening uptake, liaising with external agencies as appropriate.
7. Identify and contact low screening rates, non-responders and low participation groups.
8. Coordinating referrals and managing safety netting processes, ensuring symptoms and referrals are not missed and 2-week waits (2WW) are achieved.
9. Gathering and interpreting data to improve coding, screening, faster diagnoses, and health inequalities. Audit and review cases where patients presented repeatedly before referral and late diagnoses.
10. Drive up standards of personalized patient care by involvement in the early phases after diagnosis, ensuring referral to other key healthcare providers such as social prescribers, health and well-being coaches and other members of the practice team, and local programmes are in place.
11. Support practices to deliver their quality improvement plans for early cancer diagnosis.
12. Ensure patients receive a Cancer Care review in line with national defined timescales & targets.

Oldham Central Primary Care Network are 11 GP practices in central Oldham. As demand for healthcare increases, our GP practices now work together, with other local partners, to provide a range of extra services side by side; ones our local communities tell us they need the most.
We work together to make our resources and skills go even further and this is managed by our supportive Clinical Director, Dr. Brian Lewis and PCN cancer lead Dr. Brian Perkins.
Job responsibilities
The PCN Care Coordinator will support GP practices within the Primary Care Network, working within professional and clinical boundaries as part of an established multi-disciplinary team to deliver key objectives of the Primary Care Network DES.

Person Specification
Experience
* Experience in a patient/customer facing role
* Experience of working within a cancer-related role or primary care.
Qualifications
* GCSE grade A - C in English and Maths
* NVQ3 in Health & Social Care
Skills and Abilities
* Access to a vehicle and ownership of a full, clean driving licence
* A good standard of literacy and numeracy in line with role and duties
* Exceptional interpersonal, communication and organisational skills
* Excellent time management skills and ability to work in high-pressure situations
