Changing Futures is a national programme working across 15 areas of the country and with people who may be described as having multiple disadvantage in the areas of housing, mental health, substance misuse, contact with the criminal justice system and/or domestic abuse. For more information on the programme, please visit our website www.changingfuturessheffield.info The Role Within the role you will be working on an assertive outreach basis with a small caseload of people who are experiencing multiple disadvantage. Your role is to offer a trauma-informed, person centered approach to individuals to support them to achieve their goals which may include reconnecting to key services that they were previously disengaged from. You will be working closely with a range of agencies e.g. housing, probation, substance misuse service etc. and as part of a close and supportive team within the programme. Who are we looking for? To be successful, you may have lived experience around homelessness, mental health, substance use, contact with the criminal justice system or domestic abuse. You will also have an understanding of working with people from a range of backgrounds and how to champion trauma informed, person centered work.
