Job summary INTERNAL MPFT APPLICANTS ONLY The direct provision of high-quality evidence based person-centered physical and mental health care to patients within the Community Mental Health Pathways/Mental Health Service. To work in partnership with service users, carers and other agencies to ensure effectiveness of outcomes. To undertake expert physical health assessment, planning implementation and evaluation of care for a specific group of service users. Providing leadership in the delivery of high quality, safe and effective care to the wider multi-disciplinary team, particularly STR workers who provide input to the physical health clinics. To supervise and teach service users and carers, professionals from the multi-disciplinary teams, qualified and unqualified staff and the public. Main duties of the job Physical health outcomes in people diagnosed with serious mental illness are significantly poorer than the main population. In order to address this inequality this role will support patients who are cared for under Mental health team to undertake an annual health checks and should be offered proactive advice on how they can make lifestyle changes and improve their physical health. Working across a range of community mental health services the post holder will work closely with the teams undertaking annual physical health checks, in a clinic environment and, where this is not possible, in the patients' own home. The assistant practitioner has a breadth of knowledge and skills providing holistic and person centred care and support for people of all ages and in a variety of settings. The assistant practitioner works independently under the leadership of registered nurses/NMP's and consultant psychiatrists. The Assistant practitioner will work under the supervision from a registered nurse or other registered professional to provide high quality care and treatment as a member of the multi-disciplinary team informed by excellent communication, analysis of information, planning and organisational skills.
