A bit about the role
Are you passionate about supporting young individuals in their journey towards independence? We're looking for a dedicated Transition Worker to assist young people transitioning from care to independent living or back home. This role involves collaborating closely with youths, their families, and various agencies to ensure tailored support is provided. Responsibilities include nurturing essential life skills, facilitating access to education and well-being services, and guiding them through the complexities of adulthood.
The AB56 service aims to provide holistic, needs-led, rights-based family support to families in Moray with children aged 0-12 and a child under 5 in the household. This Tier 2 Family support service bridges the gap between universal services and targeted interventions, reducing the need for referrals to Social Work services.
The service delivery model offers preventative interventions to families in their homes and communities, helping them identify needs and develop strategies to address them. The core theme of emotional, physical, and social wellbeing guides all service activities, with a focus on keeping children safe and placing families at the heart of decision-making processes.
How you'll help to create brighter futures
Some key responsibilities of the Transition Worker role will be:
* Supporting young people living in care (inc. foster or supported living) to successfully transition into their own tenancy - or in some cases to return home
* Working with the family and wider supports, where applicable, of young people transitioning out of care
* Supporting young people living in care access appropriate services that meet their needs
* Working with the team around the young person to ensure they are supported in terms of what next
* Working with young people (not just care experienced) who are perhaps struggling to access appropriate support inc. school / learning
* Providing support across a range of scenarios using a wide range of support techniques. From organisational skills, life skills and wellbeing support.
Let's talk about you
* Experience of working with young people across a range of settings including Youth work and working with care experienced young people
* Experience of working as part of a multi-agency team and with families as well as 1-2-1 with young people
* Experience of strategic group and a partnership approach to policy and practice development
* Experience of managing cases, in accordance with agreed procedures and completing assessments, planning and reviews to ensure positive outcomes
* Ability to keep the child and or young people as the focus and ensure their feelings and wishes are represented in meetings and record keeping
