This is a great opportunity for a pharmacist to join the Pharmacy Team in South Glasgow HSCP. This exciting role will involve working with GPs and the primary care multidisciplinary teams to improve medicines management and deliver excellent patient care. This role will involve delivery of the pharmacotherapy service as part of the GP contract and will provide an important function in supporting patients with their medicines through clinics. The objectives of the team are to improve the quality and cost effectiveness of prescribing and medicines use for patients across NHS Greater Glasgow and Clyde. You will work within our pharmacy hub, GP practices, care homes and other healthcare settings to provide individualised clinical care to patients to improve the safety and effectiveness of their medicines. This will involve working closely with colleagues from a variety of professions including GPs, other practice staff, community pharmacies, care home staff, members of the primary care healthcare team and central team colleagues in supporting the development and application of high quality, safe, cost effective, rational and evidence based prescribing in the Health and Social Care Partnership (HSCP).
You will deliver excellent patient care and carry out clinics either remotely or in a GP practice setting/Care Home setting. You should have excellent communication, negotiation, organisational and IT skills, along with the ability to influence and affect change. You must have moderate post registration experience. You may have to travel between GP practices and care homes to carry out the duties of this post. Experience of directly working with primary care teams and knowledge of GP systems, as well as an independent prescribing qualification would be advantageous. This post requires PVG Scheme Membership/Disclosure Scotland Checks. You must be registered with the General Pharmaceutical Council.
