An exciting opportunity has arisen for a Hospital In-reach Lead Clinician/Nurse/ Occupational Therapist to work in Kingston Hospital for the Richmond Response and Rehabilitation Team (RRRT). The service operates from 7am until 7pm across Richmond borough.
As a Lead Clinician/Nurse/ Occupational Therapist you will assist the RRRT Service Manager in the management of a designated caseload resulting from patients you have identified as appropriate for safe and timely discharge from Kingston Hospital.

The Hospital In-Reach (HIT) Lead Clinician/Nurse/ Occupational Therapist will lead on the development and implementation of the RRRT function of in-reach to Kingston Hospital.
There will also be a key responsibility of improving patient access to overall KRFT NHS Trust community services to ensure we get it right the first time, every time.
• The post holder will work closely with RRRT Service Manager, Divisional Managers, Assistant Directors and Clinical Operational Leads to deliver the agreed outcomes.
• The post holder will need to provide the leadership and representation of RRRT at Kingston Hospital Foundation Trust meetings whilst ensuring patient pathways are supported, developed and embedded into practice.
• The post-holder will operate at all times with reference to the Trust’s objectives and quality priorities.
