Main duties of the job
The roleoffers a varied work agenda providing medicines optimisation support and expertpharmaceutical advice across the 2 practices in our network, including theimplementation and delivery of prescribing initiatives, improving prescribingsafety and provision of medication reviews, fully supported by an experiencednetwork of colleagues. The successful applicant should be able to demonstrateexcellent clinical, and communication skills and have experience ofimplementing medicines optimisation, quality and patient safety initiatives andresolving prescribing issues. You willbe a Registered Pharmacist with the General Pharmaceutical Council with aminimum of 3 years post qualification experience, who is preferably anIndependent Prescriber. You must also be computer literate, organised and beable to work independently as well as part of a team. The successful candidate will be able toprioritise workload, build key relationships and be confident interacting witha wide range of stakeholders such as GPs, Nurses, secondary care clinicians andpatients.

Primary Duties and Areas of Responsibility
Expected to work aspart of a multi-disciplinary team in a patient facing role to clinically assessand treat patients using expert knowledge of medicines for specific diseaseareas
Be a prescriber, andwork with and alongside the general practice team
Be responsible forthe care management of patients with chronic diseases and undertake clinicalmedication reviews to proactively manage people with complex polypharmacy,especially the elderly, people in care homes, those with multipleco-morbidities (in particular frailty, COPD and asthma) and people withlearning disabilities or autism (through STOMP Stop Over MedicationProgramme)
Provide specialistexpertise in the use of medicines whilst helping to address both the publichealth and social care needs of patients at the PCNs practice(s) and to helpin tackling inequalities
Provide leadershipon person-centred medicines optimisation (including ensuring prescribers in thepractice conserve antibiotics in line with local antimicrobial stewardshipguidance) and quality improvement, whilst contributing to the quality andoutcomes framework and enhanced services
Deliver a programmeof structured medication reviews to support patients to take their medicationsto get the best from them, reduce waste and promote self-care
Undertake a leadershiprole in supporting further integration of general practice with the widerhealthcare teams (including community and hospital pharmacy) to help improvepatient outcomes, ensure better access to healthcare and help manage generalpractice workload
Developrelationships and work closely with other pharmacy professionals across PCNsand the wider health and social care system
Undertake a centralrole in the clinical aspects of shared care protocols, clinical research withmedicines, liaison with specialist pharmacists (including mental health andreduction of inappropriate antipsychotic use in people with learningdifficulties), liaison with community pharmacists and anticoagulation services
Be part of aprofessional clinical network and undertake appropriate clinical supervision.

Hasan experience/awareness of the breadth of common long-term conditions that arelikely to be seen in a general medical practice.
