Neighbourhood Lead (Community Nursing Service)

We are looking for an enthusiastic individual with a personal desire to succeed and make a difference as a Community Nursing Operational Lead. As a Community Nursing Neighbourhood Lead, you will play a crucial role in leading and managing a multidisciplinary team, delivering high-quality, patient-centred care in the community. Working closely with GPs, community services, and social care teams, you will ensure effective case management and promote patient independence. The work is challenging, interesting and hugely varied, and the ideal candidate will have a strong teamwork ethic and sense of accountability, accompanied by the ability to work well under pressure and manage multiple tasks simultaneously. Experience of effective team management, including dealing with conflict is essential and forms an integral part of this role.The post covers the North East Essex area so a full UK driving licence and access to a vehicle for work is essential.
Main duties of the job
You will be required to:
1. Lead and manage a team of community nurses and healthcare professionals
2. Provide expert assessment, planning, and delivery of evidence-based care
3. Triage new referrals, manage complex cases, and support palliative care patients
4. Work closely with multidisciplinary teams to ensure holistic patient care
5. Ensure high standards of clinical practice and governance
6. Assist in developing closer working relationships/shared working between co-located teams, travelling and working at other sites as required, regularly reviewing processes and procedures ensuring tasks are completed in the most efficient way
7. Support the Community Nursing Operational Leads within the department displaying the organisational requirements of attitude, behaviour, competence and delivery, acting as a positive example for peers
The successful applicant will have an excellent manner, be fluent in computer systems and typing, be able to remain calm and work under pressure, adhere strictly to the rules of confidentiality, and have a professional manner at all times. Also, the ability to work under pressure is a vital element of this role. You will report directly to the Operational Lead for Community Nursing. This is a full-time position with the service operating 0800 - 1800, 7 days a week, offering a fantastic opportunity to develop both clinical and leadership skills in a community setting.

Person Specification
Experience
* Experience of working with professionals from other disciplines
* Experience of working as a member of a team but also able to work autonomously
* Advanced clinical skills with experience in the management of complex/long-term patient care
* Experience of managing/leading a team
* Experience of 3 years post qualification
* Community Experience
Qualifications
* Evidence of recent CPD
* RGN/RN ADULT, with current NMC Registration
Knowledge
* The concept of patient-centred care
* Importance of promoting self-care and independence
* Understanding the role of the community nurse and its purpose
* Ability to work on own initiative, assess patients competently and link with colleagues in the wider MDT to tailor plans of care
* Ability to effectively manage the workload, prioritise and delegate to manage capacity and demand
* Able to deal with conflict and manage appropriately
* Able to lead and motivate a team and implement change
* Understanding of integration
* Knowledge of the key NHS national policies & strategies
