Using a reablement approach the post holder will facilitate service user independence and empowerment as far as possible about existing medical conditions and disabilities and assist the service user to learn or regain skills with personal, domestic and social tasks as part of their therapeutic plan.
Main duties of the job

·Ensure that at all stages of a service user’s assessment and reassessment the service user and carers are kept fully informed of their rights and have access to all relevant information.
·To give holistic care to service users in the community, such as in the service users home, health centre / clinic and residential home.
·To hold a delegated caseload commensurate with the level of expertise.
·To plan and organise own delegated work.
·To participate in a duty rota to provide care for the service users to include unsociable hours.
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High-Cost Area Supplement
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735) pro rata for part time staff.

·To ensure practice is supported by evidence based practice, literature and peer review.
·To work alone at times in the clinical environment and in the community and always with access to support and support and supervision.
·To maintain statistical returns as required by the Organisation, including entering data onto the RIO system within the required time frame.
