Job summary
We have an exciting opportunity to join our District Nursingservice as a Band 5 Community Nurse based at Chew Medical Practice .
You will join a caring and supportive team of Community Nurses and Healthcare Assistants where individual development and progression is key and teamwork is at the centre. You will also work alongside the Reablement team, Community Matrons and specialist nurses in a multi-disciplinary atmosphere.
With service hours typically being am - 5 pm Monday to Friday with one in every 4 weekends. Your focus will be to offer advice and support to housebound service users in their own homes to promote their well-being and independence. You will also be offered a flexible role where you can work your own diary to provide continuity of care.
We offer an individually tailored induction programme to suit your needs and interests and to help you develop your skills to be the best they can be, as well as preceptorships for newly qualified nurses along with other Continuous Professional Development opportunities.

Main duties of the job
When I joined the community I assumed the majority of my workload would be wound care but its quite the opposite, my skills have widened massively and my confidence has grown. I love the variety each day brings and being able to see the positive changes I can make to our service users.
As a Band 5 Community Nurse for the Keynsham team, you will mainly cover the Chew Valleyarea, as well as a few villages.
To complete the duties of the role a Drivers license and access to a car is necessary.
As a Community Nurse you will:
1. Responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
2. To develop and implement programmes of care
3. Advise and recommend treatment byliaisingwith GP's and other specialists within the multidisciplinary team
4. To take charge in the absence of senior grade staff
5. Administration of medication s/c, im, pr, IV, transdermal, oral, controlled drugs, along with palliative care, wound care and IV therapy.

You will:
Be an NMC Registered Nurse or a newly qualified nurse Have a full UK driving license with access to a car for work purposes. Can work both independently and as part of a wider multidisciplinary team. Have strong communication and organisation skills and a flexible approach You will be looking for a role that is rewarding and varied and where you get to develop lasting relationships with service users.
