As a Mid-Tech Patient Care Coordinator, you'll be responsible for managing specialised therapies and providing comprehensive support to patients undergoing advanced treatments. From handling healthcare professional (HCP) queries to coordinating temperature excursions and equipment arrangements, you'll play a crucial role in ensuring the well-being of our patients. Key Responsibilities: Specialized Therapies: Manage patient queries and support for 7 specialized therapies, including ERT, Amvuttra, PAH, HPH, IVG, CF, and Parkinson's disease treatments. HCP Queries: Address queries from healthcare professionals promptly and effectively, providing accurate information and support as needed. SDD Arrangements: Coordinate special delivery details for patients receiving mid-tech treatments, ensuring timely and accurate deliveries. Urgent Queries: Handle urgent patient queries with efficiency and empathy, ensuring timely resolution and patient satisfaction. Ancillary Queries: Assist patients with ancillary queries related to their treatments, providing clear information and guidance. Temperature Excursions: Monitor and manage temperature excursions for medication and equipment, taking appropriate action to maintain product integrity. Equipment Arrangement : Coordinate arrangements for equipment such as pumps and refrigerators, ensuring patients have everything they need for their treatments. Collections : Facilitate the collection of equipment or medication as needed, coordinating logistics and ensuring smooth transitions. Inbound and Outbound Support: Handle both inbound and outbound communication with patients, providing support and assistance as required. Manage Shared Mailboxes: Monitor and manage shared mailboxes efficiently, prioritising and addressing incoming communication promptly. Liaise with Nursing: Collaborate with nursing teams to ensure seamless coordination of patient care and support. PV's/PC's/Safeguarding/Complaints : Handle patient verification (PV), patient complaints (PC), safeguarding issues, and other related matters professionally and empathetically.
