Community MH Nurse (OA) Rapid Intervention and Treatment Team (RITT)

Fylde Coast RITT is a crisis Older Adults Community Mental Health service based at the Lytham Unit, Lytham Hospital.
We are a multi-disciplinary team, with close working links with other health departments, social services and voluntary agencies.
Our service users can present with both functional and organic mental health difficulties which are often combined with complex social care and physical health needs.
RITT has 2 core functions:
1. Home treatment: The provision of care coordination for patients who are experiencing acute mental health difficulties and who are presenting as high risk to themselves or others in the community and so require intensive home support from community services. Conducting Gatekeeping assessments for mental health inpatient admissions is a core component of this role as is facilitating early discharge from inpatient units.
2. Care Home Liaison Team: Providing support for service users and carers for people who live in a residential home and are presenting with Behavioural and/or Psychological Symptoms of dementia.
We are looking to recruit a Band 6 Community Mental Health Nurse who has an enthusiasm for and passion to work within older adult's mental health services.
The Band 6 Nurse will have the opportunity to consolidate their skills in mental health nursing as well as developing the specialist skills needed in older adults mental health.
Main duties of the job
1. To actively promote teamwork, respect, integrity, accountability, excellence, and compassion within the clinical team in accordance with the Trust values and code of conduct.
2. To provide nurse leadership within the multidisciplinary team and ensure the effective assessment, formulation, planning, and monitoring of care given to service users.
3. To undertake mental health assessments including Risk assessments within a multidisciplinary assessment team.
4. To provide a fast, responsive service to assess individuals' needs.
5. Providing a communication structure between GPs, IAPT, and non-IAPT services in LCFT, non-statutory service, as a means of ensuring the highest possible standards of patient care.
6. To provide mentoring and clinical supervision to other staff and trainees on placement.
7. Where the post holder is a non-medical prescriber, practice responsibly and maintain competence to effectively prescribe from the relevant prescribing formulary.

We are a 7-day a week service with operating hours from 8am to 8pm, shifts worked at present can be Early (8-4pm) Late (12-8pm), including weekend working. Flexible working can also be considered to work 4 x 9.5 hour days (8am - 6pm / 10am - 8pm daily).

Education/Qualifications
* Registered Mental Health Nurse Degree
* On-going registration with the NMC
* Specialist qualification or equivalent level in skills and development
* Evidence of commitment to continuing professional development
* Mentorship qualification
* Honour degree 2:2 or above or an equivalent professional qualification related to Health
* Sign-Off Mentor qualification
* First line management/leadership qualification
Experience
* 2 years minimum experience at Band 5
* Post graduate experience
* Supervision and management of staff
* Able to work as part of a multidisciplinary team
* Experience of assessing, planning, implementing, and reviewing care needs
* Experienced in acting as a mentor/assessor of student staff
* Experience of CPA Care Coordinator role
